of Adoption Certificate If a dog comes into our
facility pregnant, then we start the puppies off
from the minute they are born with human scent
imprinting, and early neurological stimulation. If
it is a litter we have chosen, from a
breeder, they are bred specifically for our
service dog and therapy dog for children and
schools program. All dogs presently posted were
born in the house, raised in the house, introduced
to Early Neurological Stimulation at birth, and
have been socialized with kids from age 2, age 4
age 8 and older. They have been raised with cats,
handled daily, and introduced to
different textures. We are looking for service dog
and therapy dog/pup potential based on the Volhard
Puppy Temperament Test at the age of 8 weeks. The
ones that have been posted did not test for
potential service or therapy work and are being
placed in companion homes. Adoption Donation
Contract This agreement is entered into this date
by and between Pawsibilities Unleashed (Paws) and
the applicant (Adopter) for the adoption of the
above identified dog (the dog). This is a tax
deductible donation to our 501(c)3 Non-Profit, EIN
#47-4272326. Your donation goes to support our
Prison Canine Training Program and our Service Dog
Training Programs for children and
school facilities, and the upkeep of our training
facility. I will be given a tax deduction receipt
with my vetting packet.                          
   As Is Adoption TERMS AND CONDITIONS  1. Adopter
agrees to pay an Adoption Donation Fee of
$            for the above named dog.  2. Adopter
agrees to put a deposit on the dog, which bars
anyone else from applying, until potential adopter
decides for sure this is the dog for them. 
3. Deposit is 100% refundable if you decide to not
adopt the dog. If you do adopt, the deposit is
applied to the adoption donation fee.  4. Adopter
completed an adoption application which is
attached and Incorporated herein as a part of this
Agreement.  5. As of this date of adoption, the
Adopter agrees to take ownership of the above dog.
This includes all medical/vet care, medication,
food, grooming and housing care needed by the dog
beginning at the time of physical transfer of the
dog.  6. Adopter agrees to take the dog to a
licensed vet within 5 days of adoption date for a
physical and checkup at the adopter’s
expense.  7. Should a medical need arise in the
first 5 days, either a new issue, or one we
missed, Adopter agrees to either pay for the vet
care themselves or return the dog to Pawsibilities
for a refund of the Donation Adoption Fee. The
medical issue must be documented by a licensed vet
and paperwork given to Pawsibilities.
Pawsibilities does NOT pay for, nor reimburse for
any veterinary services performed outside of our
program.  8. Refunds are given in the first 5 days
from the Donation Adoption Date for medical
or behavioral issues only.      9. Pawsibilities
makes no guarantee in reference to the health
and/or temperament of the dog. The dog is
adopted, “as is” and the adopter assumes all
responsibility for treatment of any and all
existing conditions, or any other conditions of
physical or temperament changes that may occur. 
10. Adopter understands that it is the practice
of Paws not to place a known biter or know
aggressive dog. However, Adopter is fully aware
and does understand that any dog may bite or
aggress if provoked, scared, sick or injured. If
the dog shows any in-appropriate aggression
towards, food, toys, being touched, grankids,
etc., then the dog is to be returned to
Pawsibilities Unleashed for a full refund within
5 days of adoption. 11. Adopter understands that
if at any time they no longer want this dog, they
are to return it to Pawsibilities Unleashed and NO
REFUND WILL BE GIVEN - you gave a Donation - you
did not buy anything.  12. There is a $75.00 fee
for any checks returned by issuer’s bank for
any reason. This must be paid within one week of
being notified of the returned check. I understand
that this dog(s) was not bred by this facility,
nor has any prior connections to this facility,
before the intake date on their paperwork. I
understand that most of the dog(s) in this program
are owner turn-ins, breeder-owner turn-ins, and
dogs from other
rescue/shelter/humane societies/side of the road,
and dogs that were bred specifically for
our service dog and therapy dog program for
children. I understand that this facility looks
for dogs that can fill the applications they have
for service dog(s) or therapy dog(s) and that is
the top priority. Any dog that does not meet the
qualifications for this work, is placed as pet
quality, or any dog that does not fit into an
application need (wrong size for example, person
needs lab size and all we have is 25 lb dog). All
medical paperwork will be sent for your review
before you come to get the dog. If you have
questions, you can call our vet, Dr. Scott Neives,
DVM, 502-223-7277 and he will be glad to answer
them. If you are sure/fairly sure you want the
dog/puppy, you can put a deposit down to move it
to adoption pending while you decide. If you decide
this is not the dog for you upon meet & greet, we
will 100% refund your deposit and re-post the
dog/pup. If you would like the dog shipped to you,
we do have ground transport available and we also
ship by Jet-A-Pet, and you would be responsible
for shipping crate, (crate based on weight
of crate & dog), and the dogs flight ticket (based
on dog weight combined with kennel weight). We
would pay for dog up front to Jet-A-Pet, and you
would reimburse these cost to us before the dog
fly's out of the airport. If you would like your
dog crate trained, house trained, obedience,
manners, self-control, grooming & husbandry
skills,we do this through our Prison Inmate Dog
Training Program and it is a $2,500.00 donation to
keep the program going, feed the dogs, vet the
dogs, toys, treats, etc., Training last 8 weeks
approximately. Dog is assigned to a team
consisting of 2 trainer's that live with your dog
in the doggie dorm, 24/7. Dogs have a huge play
yard completely fenced with training classes
daily. If you have a puppy it will earn the AKC
Puppy S.T.A.R. Certificate while in the
program. If your dog is older, it will earn the
AKC Canine Good Citizenship Certificate. Your
paperwork will be sent to you so you can register
this with the AKC. No shock collars, prong
collars, e-collars, choke chains, pinch collars
are used in our training programs. If you would

as an adult? Take the pup's weight in pounds,
divide by their age in weeks, then multiply that
number by 52 weeks in a year. This should predict
your pup's ideal weight as an adult.
